Click in the box for the quantity of each booklet, type in the number required, then click outside the box. The amount due is calculated automatically, including the appropriate postage and packing for the postal zone.

When you have completed the form, please click the 'Check Details' button below. To start again, click 'Clear the Form'. If you get the OK message, click on the Printer icon on the toolbar near the top of the screen to print out the page. Alternatively click on 'Print' in the 'File' menu at the top of screen.

Post the completed form with a cheque for the due amount IN UK POUNDS payable to A. King. Please make sure you select the correct postal zone for the country you live in. Underpaid orders will NOT be processed. Overseas customers who wish to pay in any currency other than Sterling should ring their bank for the daily conversion rate add the equivalent of £5 in your local currency to the overall cost of the transaction, (SORRY - but - King's Medical incurs bank charges of £5 for every non-sterling cheque).
